Major Palliative Care Providers Partner with HealthConnect.Link

Madison, January 9, 2017 – HealthConnect.Link, a Madison-based non-profit venture, is very pleased to announce the recent establishment of community partnerships with Agrace HospiceCare and Home Health United, both nonprofit, community based healthcare leaders serving South Central Wisconsin.

HealthConnect.Link is creating an online community of free and subsidized health resources, helping low income uninsured or underinsured patients access needed services and support

For over 35 years, Agrace has served as a nonprofit, community based health care agency dedicated to providing expert physical, emotional and spiritual support to patients and families during the stages of serious illness and end of life.  Agrace is nationally recognized for excellent comprehensive services provided in patients’ homes, the Agrace inpatient unit, and skilled nursing and assisted living facilities throughout south central Wisconsin. 

Home Health United has a long and storied history in Dane county.  Starting in 1908, when the Attic Angel Association sent the first visiting nurse into patient homes, through the emergence of the Visiting Nurse Service in 1951, to 1999 when they merged with Home Health United.  Today Home Health United is a leader in home health care, recognized as a HomeCare Elite Agency for eleven years in a row.  As part of its mission to promote optimal health and quality of life through exceptional care, Home Health United provides a comprehensive line of services including home health, palliative care, hospice, home medical equipment, infusion pharmacy services an community programs such as SAFE at Home and Meals On Wheels.
